You touch a creature, and that creature must succeed on a Wisdom saving throw or become cursed. The curse is permanent, but can be removed either by a wish spell or if the conditions to break the curse have been met.  Remove curse cannot remove this curse (unless the DM states as such), but can, if the DM states reduce its effect for 12-24 hours. 
You can choose the standard  options from the basic remove curse spell:

  • Choose one ability score. While cursed, the target has disadvantage on ability checks and saving throws made with that ability score.
  • While cursed, the target has disadvantage on attack rolls against you.
  • While cursed, the target must make a Wisdom saving throw at the start of each of its turns. If it fails, it wastes its action that turn doing nothing.
  • While the target is cursed, your attacks and spells deal an extra 1d8 necrotic damage to the target.

Alternatively, you can choose from the following options

  • Target an ability score (Str, Dex, Con,Int,Wis or Cha), that score is permanently reduced by -2.
  • Target a skill (Stealth, Athletics, Persuasion etc), that skill is permanently reduced by -3
  • Target a class or racial ability, such as a halfling's lucky feature, sea elf's amphibious feature or fighters multiattack,. That ability is removed (DM be VERY careful!! Advised only as a story element as part of punishment or ancient curse.)    

These curses are permanent until either a Wish Spell is used, or until the conditions made to break the curse have been met. A condition must be set out and can be as vague or detailed as possible.
Example:  A rogue has lost -3 to stealth, the condition set out my be they have to return a dragon's treasure to its hoard and face the dragon's judgement. After doing so, the curse is lifted. Or, a Cleric has lost their use to channel divinity, the condition must be to pray to a rival god on the last light of summer when a hawk eats a snake. 

Only one curse can be bestowed on a single creature

Advisory: DM's, it is advised not to go overboard and use as part of a questline. Give clear and detailed instructions as to how to remove and provide a clear method for the mission. Be careful not to outright kill the PC ability to play such as removing a monks ki, ir a paladins divine smite. Do not let the curse drag out for too many sessions without player approval. Discussion advised.
