If so, the answer is no. In D&D, a spell only does what the spell description says it does. This one doesn't say anything about poisoning food, so you can't use it for that.

rot and disease in this context CAN be synonymous.
but most of the time, they are not.
you have a steak, the steak was left out too long, meaning it is rotten.
an assassin poisoned the steak.
the steak due to being left out too long, also has botulinum bacteria, a disease.
you cast purify food and drink on the steak, it still has botulism.
this spell is not good if you are dealing with food or drink that actually risk disease, such as historically speaking, milk, which spoiled fast, and also was unpasturized.

Posted Mar 23, 2026I think what they were getting at is that some previous editions of D&D had the concept of "reversible spells", spells that could be used for two opposite effects. For example, the same spell could either cause or remove blindness, or heat or chill metal, or unlock or lock a container.
This is whole concept no longer exists in 5e/5.5e D&D; the last vestige of it is the spell Enlarge/Reduce.

Posted Mar 25, 2026You still see that in cantrips, for example Control Flames lets you brighten or dim fire, and Prestidigitation lets you clean or soil surfaces, and chill or warm food. Though I suppose you’re right that you don’t really see this concept used outside of certain utility cantrips.
