Through mastery of necromantic magic you can temporarily curse the features of one target and disfigure them in a gruesome way. This disfigurement if purely cosmetic and aside from a slight itching sensation the victim of this curse is unaware of the effect until such time as they see their reflection or others notices it.
For the duration of the spell, any living creature that can see the Disfigured victim must make a wisdom saving throw against your spell DC. On a success they are shocked and repulsed but otherwise act normally and cannot be affected by this spell for the rest of its duration. On a failed save anyone viewing the disfigurement is subjected to the Frightened condition. Should the cursed victim see their own reflection then they must also make a wisdom saving throw or become frightened of their own reflection.
A second use for this is for you to curse yourself, if you do you can choose to either effect others with the above frightened effect or you can choose to give yourself a disfigurement that grants you advantage on Intimidation ability checks for the duration of the spell.
A creature frightened by this spell can repeat the saving throw at the end of each of its turns, on a success the creature is no longer frightened and cannot be frightened again by this spell for the rest of the duration.
