You hold out the blade towards the target creature, and it transforms into a small smoky cloud in the shape of a dagger. It surges forward through objects and people before it meets it's target, impaling itself into their chest (if they have one). The target must make a dexterity saving throw using your spell save score, on a fail the target takes 12d6 necrotic damage.
