You attempt to create an artificial silver cord to tie a soul to a vessel.
You touch a dead creature that has been dead for no more than a century, that didn’t die of old age, and that isn’t undead. If its soul is free and willing, the target returns to life with all its hit points.
This spell neutralizes any poisons and cures normal diseases afflicting the creature when it died. It doesn’t, however, remove magical diseases, curses, and the like; if such effects aren’t removed prior to casting the spell, they afflict the target on its return to life.
This spell closes all mortal wounds and restores any missing body parts.
Coming back from the dead in an unfamiliar body is an ordeal. The target takes a −2 penalty to all attack rolls, saving throws, and ability checks. After the target finishes a long rest, the penalty disappears.

There is a chance that if the soul is tied to a corpse, they become a reborn. The DM decides on the percentage of damage to the body, and then rolls a d100. If the roll is lower than the percentage, the soul comes back as a reborn.
If soul is tied to an object such as a weapon, it becomes a sentient object and retains some of the abilities that it had in it's past life. The DM decides what these are.

A filled Soul Cage or a similar soul-trapping vessel is required when using this spell on an unwilling soul. If using soul cage, you must use all 6 charges or the soul will be able to resist by rolling a charisma save against your spell save dc. For each missing charge, the soul gains +2 to their roll. If it fails, the soul escapes but the components aren't consumed.

* - (Metals or gems worth 1,000 gp which the spell consumes, body or item to act as vessel, item of value to soul.)
