Blood spear

The blood of the caster is drained out of them, taking the form of a small spear in their hand. The caster takes 1d6 damage, then makes a ranged spell attack roll. If the spell hits, the target takes 3d12 force damage.

At higher levels: the force damage increases by 1d12 per spell level above 1st, and the damage taken by the caster increases by 1d12 for each level above 1st
