You make a melee spell attack agaisnt a creature or object you can reach. You can use a weapon or your own body to attack. On a hit, the target has to succeed a Constitution saving throw. If the target succeeds it will take 4d10 necrotic damage. If the target fails the Constitution save, it will have part of it's body cut off without taking any damage. The target's body and the cut off part function exactely the same as before as if nothing had happened, meaning the target can still use the separated bodypart. An affected creature can't regenerate the cut off part and it can't be healed by magic, because there is no damage to heal. The spell can be broken by a remove curse spell.
The duration of the spell is permanent until it is broken or the separated body part touches the corresponding other end again. Forcibly disabling the spell will cause the body parts to lose their connection.
