You touch a living creature, whispering a call to your goddess, leaving a thin coating of spores on it's flesh. It takes 3d10 poison damage (with a Constitution save for half damage. If the creature dies due to damage from the spell or within the next minute, it rises as a Fungal Corpse under your control.
The Fungal Corpse is friendly to you and your companions. In combat, the creature shares your initiative count, but it takes its turn immediately after yours. It obeys verbal commands that you issue to it (no action required by you). If you don’t issue any, it defends itself but otherwise takes no action. You may control no more than four Fungal Corpses, and any created beyond the initial four will become uncontrolled.
At Higher Levels. When you cast this spell using a spell slot of 5th level or higher, the creature assumes the higher level for that casting wherever it uses the spell’s level in its stat block, and the damage is increased by 1d10

Undead; size based on original creature, neutral evil
Armor Class 11 + the level of the spell (natural armor)
Hit Points equal the undead's Constitution modifier + your spellcasting ability modifier + ten times the spell's level
Speed 30 ft.
Damage Immunities necrotic, poison
Condition Immunities exhaustion, frightened, paralyzed, poisoned
Senses darkvision 60 ft., passive Perception 10
Languages Common, understands the languages you speak
Festering Aura: Any creature, other than you or your companions, that starts its turn within 5 feet of the undead must succeed on a Constitution saving throw against your spell save DC or be poisoned until the start of its next turn.
Actions
Multiattack. The spirit makes a number of attacks equal to half this spell’s level (rounded down).
Rotting Claw (Putrid Only). Melee Weapon Attack: +3 + the spell’s level to hit, reach 5 ft., one target. Hit: 1d6 + 3 + the spell’s level slashing damage. If the target is poisoned, it must succeed on a Constitution saving throw against your spell save DC or be paralyzed until the end of its next turn.

Interesting concept but I feel like it could use some alterations. For example, the hit points of the fungal corpse are ridiculously high for something created by 4th-level spell and only get higher with later spell levels so I recommend lowering them. On the contrary, the fungal corpse's damage output is surprisingly low and even the paralyzed debuff doesn't quite make up for that. Still an interesting spell, though. You have my upvote.
This is the kind of spell that I hope would inspire other would-be creators. It's extremely well thought out, the design is impeccable, and above all else, it's just plain creative.
