You pull the essence of souls of two targets and force them to switch their mortal host. After the casting time each target must make a wisdom saving throw if they are unwilling. If one of the bodies were to perish the spell becomes permanent and the soul is lost.
You use the STR, DEX, CON of the new target as well as any spells they might have prepared or know. The spell may end early if the targets where to maintain contact for one minute and both are willing.
Magic items that are attuned to one target are still attuned to the same target. If one target is on a different plane of existence as the other target the spell becomes permanent. Your voice is the same as well as any muscle memory that the target may have such as simple weapon usage or spell casting.
