You touch one willing creature as they are about to make a mistake on a skill check. If the caster successfully rolls a Wisdom saving throw against a DC equal to 10 plus the target's skill bonus, the target can roll a d4 and add the number rolled to the skill check. 

The original version of this spell is too annoying to administer in actual gameplay.
