You touch a dead humanoid or a piece of a dead humanoid. Provided that the creature has been dead no longer than 10 days, the spell forms a new adult body from the old peice(s) or body for it and then calls the soul to enter that body. If the target's soul isn't free or willing to do so, the spell fails.
The magic fashions New parts or in more extreme cases almost a whole body for the creature to inhabit, which likely causes parts ot the creature's race to change. The GM rolls a d100 and consults the following table to determine what form the creature takes when restored to life, or the GM chooses a form.
Note: this is a variant rule where only the damaged body parts will grow as the new race. For example a human who lost their head may come back as a human with a dragonborn head. They can optionally gain or lose the traits related to the new or old parts ex: breath weapon, darkvision, natural weapons. And the ability score increase changes to whatever race is more present.
You may use this normal table or the variant table with updated races.

The reincarnated creature recalls its former life and experiences. It retains the capabilities it had in its original form, except it exchanges its parts of its original race for the new one and changes its racial traits accordingly. Most mixed racial traits may require further homebrew.
