You target one creature within 120 feet that has a normal bone structure. That creature must succeed on a Constitution saving throw. On a failure the creature is incapacitated until the end of it's next turn, takes 45 (10d8) piercing damage, and if it fails by five or more, must roll on this table. On a success, the creature takes 36 (8d8) piercing damage and suffers no other effects.
1. Organ damage. The target takes another 36 (8d8) piercing damage and make another save. On a success the creature suffers no adverse effects and on a failure the creature is stunned for one minute. If it fails the save by five or more it dies.
2. Limb loss. The target loses 1d4 of it's limbs, or at least cannot use them.
3. Spine/brain damage. The creature must make another save. If it fails the save it dies. On a success the creature suffers major brain damage. Work with your GM to find out the effects, eg always is under the equivalent of a confusion spell, permanently paralyzed, loses control of a limb, forgets everything.
4.Muscle damage. The target takes an additional 9 (2d8) piercing damage and loses 1d4 points of strength.
