You change a part of your own physical form. You can perform a combination of three of the following until it is dispelled:

  • You alter the appearance of two of your limbs (in sets, ex: two arms, two legs). 
  • You alter the appearance of any associated parts (ex: wings, additional heads, halos or other aureoles, horns).
  • You alter the appearance of your eyes, ears, nose, etc. 
  • You alter the appearance of your torso. 
  • You alter the appearance of your neck.
  • You alter the appearance of your hair.
  • You alter the appearance of your height. 
  • You alter the appearance of your weight.
  • You alter the appearance of your hands or feet.

The effects are minimal. Changes can be made in color, size, and shape, but only to a certain extent: 

  • You cannot add or remove body parts; however, invisibility is possible.
  • You can change parts of your body into weapons (ex: arms into swords) but they would attack as if they were in their original form. 
  • You can change parts of your body in relation to the environment (ex: webbed fingers while swimming) but they do not improve you (ex: swimming speed).
  • You cannot alter your eyes to enable darkvision.

A creature can use its action to inspect your appearance and must succeed on an Intelligence (Investigation) check against your spell save DC. On a succes, the creature knows what parts of your body are being altered and how. All changes can be dispelled at once, or individually. 

Simply, this is a party trick, and is not generally useful during battle or while in dangerous situations (that being said, the DM can make decisions based on the desires of the PC, as long as they stay within the limitations of the spell). The DM is free to reasonably add any other areas that can be changed on a player's body.
