Base Class: Barbarian
Most barbarians unleash their rage through sheer strength and fury, but you have become something else—something monstrous. Whether through an ancient curse, a pact with an unknowable entity, or exposure to forces beyond mortal understanding, your body has begun to change. At first, it was subtle—your skin toughened, your nails sharpened, your muscles swelled beyond their natural limits. But as time passed, the changes became more severe. Your bones shift and crack, growing in unnatural ways. Your voice distorts into something inhuman. Your very presence unsettles those around you, for they can sense the horror that lurks beneath your flesh.
You are no longer simply a warrior—you are a living nightmare, a creature of rage and mutation. Each battle fuels your transformation, pushing you further from what you once were and closer to something truly terrifying. Some barbarians who walk this path embrace their monstrous evolution, reveling in their new form and the fear it instills. Others fight desperately to hold onto their humanity, even as it slips away like sand through their fingers. But one thing is certain: once you set foot on this path, there is no turning back.
Aberrant Metamorphosis
At 3rd level, when you enter a rage, your body mutates unpredictably, twisting in response to the battle. Roll a d4 and gain the corresponding mutation until your rage ends. You can also choose to automatically manifest one you’ve rolled before instead of rolling.
1 - Hunger Maw: Your jaw distends into a horrific maw. You gain a bite attack (1d8+ Strengh Modifier) and regain HP equal to half the damage dealt when you hit with it.
2 - Chitinous Carapace: Your skin hardens into unnatural plating. You gain +1 AC and resistance to force and thunder damage.
3 - Mutant Appendage: You sprout an additional limb, tail, or tendrils, allowing you to make an extra unarmed strike (1d6 damage) as a bonus action.
4 - Rending Strikes: Your nails as sharp as the claws of the monster you are. Your unarmed strikes deal 1d6 + Strength modifier slashing damage instead of bludgeoning damage, and ignore resistance to slashing damage.
Additionally, your warped form unsettles those around you. You gain advantage on Intimidation checks, but disadvantage on Persuasion checks.

Flesh Unbound
At 6th level, your body no longer follows the laws of nature, reshaping itself at will. You gain the ability to forcefully mutate as a bonus action while raging, replacing your current mutation with another from the Aberrant Metamorphosis table. You can do this once per long rest.
Additionally, you gain one of the following permanent adaptations of your choice in exchange for another one effect of the Aberrant Metamorphosis table:
- Shifting Flesh: When you take damage, you can use your reaction to reduce it by 1d12 as your body twists to absorb the blow.
- Barbed Growths: When a creature hits you with a melee attack, they take piercing damage equal to your Constitution modifier.
- Tendril Strike: You can grapple creatures up to 15 feet away. While grappling a creature this way, you can use a bonus action to pull it 10 feet closer to you.
Nightmare Incarnate
At 10th level, your presence warps reality, turning you into a living terror. Creatures of your choice within 10 feet must succeed on a Wisdom saving throw (DC = 8 + your proficiency bonus + your Constitution modifier) when you start your rage, or they become frightened of you for 1 minute. They can repeat the save at the end of each of their turns.
Additionally, your mutations become more potent. When you roll a mutation at the start of your rage, you now roll a d6 instead of a d4, gaining access to two new effects:
5 - Warped Blows: Once per turn, when you hit a creature, another creature within 10 feet takes psychic damage equal to your Constitution modifier.
6 - Unstable Evolution: Roll twice on the mutation table and gain both effects.
Additionally, your eyes deform as your mutation grows. You gain 60ft of Dark vision to hunt at every time in every cave and everyone's darkest nightmares.
Abyssal Rebirth
At 14th level, your transformation is complete—you are no longer bound by mortal form. When you rage, you undergo a horrifying final evolution, choosing one of the following effects:
- Wings of the Profane: You sprout monstrous wings, gaining a flying speed of 20 feet.
- Regenerative Horror: At the start of your turn, if you have less than half your HP, you regain hit points equal to your Constitution modifier + your proficiency bonus.
- Dread Unleashed: When you enter a rage, all creatures of your choice within 30 feet must succeed on a Wisdom saving throw or be frightened for 1 minute.
- All-Consuming Hunger: Your bite attack now deals 2d10 damage, and when you hit a creature with it, you gain temporary HP equal to the damage dealt.
Additionally, you no longer age, and your type becomes Monstrosity instead of Humanoid. Those who look upon you whisper in fear, for you are no longer of this world—you are something else entirely.
