Base Class: Rogue

Inspired by the Grim Surgeon from the Griffon's Saddlebag. For D&D 2024.

Bloodbone Rogues are uniquely familiar with the workings of a body. They see the patterns of veins and skin that every creature is built from, and exactly where to target - whether for harm or healing. Whether doctors from rough or war-torn backgrounds, or assassins with a less than healthy obsession with what they kill, bloodbone rogues are studied in life and death, and have learned to manipulate them as one.

Level 3: Transfuse

Starting at 3rd level, you can manipulate the blood drawn from your incisions, healing as you damage enemies. Once on each of your turns when you deal Sneak Attack damage to a creature that isn’t a construct or an undead, and has a CR of greater than 0, you can expend one of the Sneak Attack dice to instead heal an ally of your choice within 10ft of the attacked creature. You roll 1d6 less damage for the Sneak Attack, and the ally regains hitpoints equal to your wisdom modifier. 

Once you reach 8th and 15th level, this feature heals by twice and thrice your wisdom modifier respectively.

Level 3: Surgeon

Studied in the ways of the body, you gain proficiency in the Medicine skill, and your proficiency bonus is doubled for any ability checks you make using this skill. 
In addition, you can manipulate the blood of your allies, enabling you to stabilize any unconscious creature within 5ft as a bonus action, without the use of a healer's kit. 

Level 9: Precision Incisions

Your study of blood and wounds grants you another way to use your sneak attack. When a creature is Bloodied, you don't need advantage or another ally within 5 feet of it to use Sneak Attack against it.

Level 13: Exsanguinate

When you heal an ally via your Transfuse feature, the target of the attack is weakened from the blood loss. Until the end of your next turn, the creature's speed is reduced by 10 ft and the creature is considered as bloodied, regardless of its hitpoint level.

Level 17: Blood Surge

At 17th level, you can further manipulate the blood you spill. Immediately after you deal Sneak Attack damage to a creature, you can draw every litre of blood spilt into another creature within 30ft of the target, healing them for the full Sneak Attack damage dealt, and reducing the targets movespeed to 0 until the end of its next turn.

Once you use this feature, you can’t use it again until you finish a short or long rest, and you cannot use this feature against a construct or undead.
