These calculating predators spend years studying the body's vulnerabilities with scientific dedication, memorizing every nerve cluster, pressure point, and arterial pathway until they can end a life with mathematical certainty. Where other rogues see targets, Death Surgeons see intricate maps of mortality, each weakness a doorway to instant death. They cultivate supernatural patience, training to remain perfectly still for hours while reading the rhythm of combat, waiting for the precise moment when an enemy's defenses reveal the pattern they've killed a thousand times before.
In battle, a Death Surgeon is a study in lethal contradictions. They may stand statue-still as chaos erupts around them, ignoring easy opportunities while waiting for the perfect strike - then suddenly commit their entire being to a single attack that cannot miss, cannot fail, and cannot be survived. When conditions align and enemies weaken, these methodical killers transform into something else entirely: dealers of cascading death who leap from victim to victim, each kill enabling the next in a brutal chain reaction that can clear a battlefield in heartbeats. To face a Death Surgeon is to face someone who has reduced your existence to a solvable equation - and they always show their work.
Anatomical Precision
3rd-Level
Your intensive study of anatomy has transformed you into a methodical killer who exploits vulnerabilities others cannot see. Through years of studying pressure points, nerve clusters, and vital organs, you've developed techniques that turn every strike into a potential death blow.
You can use your Sneak Attack without advantage when targeting anatomical weaknesses under these conditions:
• Isolated Target: No other creatures are within 5 feet of your target
• Patient Strike: You haven't moved this turn and cannot move after the attack
• First Blood: The target hasn't taken damage yet this combat
• Wounded Prey: The target is below half its hit point maximum
You still cannot use Sneak Attack if you have disadvantage on the attack roll.
When you commit completely to a single strike, using both your action and bonus action to make one weapon attack, your anatomical knowledge ensures lethal precision. This committed assault automatically qualifies for Sneak Attack (even if you have disadvantage), deals additional damage equal to your rogue level, scores a critical hit on a roll of 19 or 20, and prevents any movement before or after the strike.
Additionally, whenever your Sneak Attack finds its mark, you can target specific pressure points to inflict one of the following effects until the start of your next turn: reduce the target's speed to 10 feet (Cripple), prevent the target from taking reactions (Disrupt), or grant advantage to the next attack roll against the target (Expose).
This medical precision extends beyond combat. You gain proficiency in either Medicine or Insight (your choice), representing your deep understanding of how bodies work—and how they break.
Pre-operative Assessment
9th-Level
You've learned to read subtle cues in battle and anticipate your foes. As a bonus action, you can mark one creature you can see within 60 feet. Until the end of your turn, attacks you make against that creature qualify for Sneak Attack, even if you would normally have disadvantage.
You can use this feature a number of times equal to your proficiency bonus per long rest.
Operating Theater
13th-Level
Your movements become fluid and unpredictable:
- Opportunity attacks against you have disadvantage
- You can move through enemy spaces (but can't end there)
- Once per turn, when you hit with a Sneak Attack, you can teleport up to 10 feet to an unoccupied space you can see
Chain Dissection
17th-Level
Your lethal precision creates a cascade of death. Whenever you reduce a creature to 0 hit points with a melee weapon attack, you can immediately leap to another enemy within your jump distance and make another melee weapon attack as part of the same action. This attack can benefit from Sneak Attack, even if you've already used Sneak Attack this turn.
This effect can trigger multiple times in the same turn - as long as you keep reducing creatures to 0 hit points and have valid targets within jump range, you can continue the chain dissection.
Jump distance = Strength score in feet (or half that without a running start)
