Alex Mercer was the first. 

Warriors of Affliction are the prototypes for the next steps in human evolution. These Warriors have been infected with a virus which causes rapid mutations. The experiments were meant to solve the worst of humanities diseases, it appears it may have produced something much worse.

Level 3: Affliction Manifest

Denial and Anger. You gain the following benefits.

Alien Flesh. As a reaction to rolling initiative or as an action on your turn you can spend 1 focus point to manipulate your defenses. You gain one of the following benefits for 1 hour.

Disguise. You make yourself—including your clothing, armor, weapons, and other belongings on your person—look like a creature within 5' of you. You can seem 1 foot shorter or taller and can appear heavier or lighter. You must adopt a form that has the same basic arrangement of limbs as you have. To discern that you are disguised, a creature must take the Study action to inspect your appearance and succeed on an Intelligence (Investigation) check against your Monk DC.

Shield. You gain a bonus to your Armor Class equal to 1/3 your Monk level rounded down. 

Alien Armaments. As part of the bonus action to Flurry of Blows you can change your arm(s) into deadly armaments. The transformation last for 1 minute unless you use this ability again or spend a bonus action to return your arm(s) to normal.

Blade. Enemy creatures within 5' of a creature struck by Blade take damage equal to your Dexterity or Wisdom modifier (whichever is higher).

Claws. When you take the Flurry of Blows action on your turn with Claws, you can make an extra attack. 

Whipfist. You have a reach of 15'. When you strike a creature with Grapple, you can immediately move yourself within 5' of the creature at the cost of no movement. 

Level 3: Otherworldly Vision

Bargaining. You gain the following benefits. 

BlindsightYou gain blindsight out to 10'. 

Sense the AfflictedYou gain advantage on Perception and Survival checks used in tracking and perceiving Undead, Monstrosities, and Abberations. 

Level 6: Not of this World

Depression. 

Disguise. Now persists until a long rest. As a bonus action, you can switch between your Disguise and your normal visual form. When you take a Disguise you can choose to make it appear distorted causing the Uncanny Valley effect. If you choose to do so, you have disadvantage on Persuasion and Deception checks but advantage on Intimidation checks. 

Glide. When you fall at least 10 feet above the ground, you can use your reaction to glide horizontally a number of feet equal to your walking speed, and you take 0 damage from the fall. You determine the direction of the glide.

Level 17: Molecular Devestation

Consume all. 

As an action, you can force all enemy creatures within 30' of yourself to make a Constitution Saving throw. Creatures take force damage equal to 5 rolls of your martial arts die on a failure, half on a success. You gain a number of temporary hit points equal to the rolled damage + your Monk level. 

You can use this ability once per long rest, or use 10 focus points to regain one use of the ability.
