Base Class: Cleric
House Jorasco is celebrated across Khorvaire for its healing prowess, but no House rises to such heights without keeping knives hidden in the shadows. The Nosomatic Surgeons are those knives. Masters of disease and decay, they manipulate contagion with surgical precision, customizing illness to cripple, silence, or slay those who threaten the House’s interests.
No Nosomatic Surgeon will ever admit their true allegiance; the work they do is deniable, and always must be. Discovery is not an option—better to die in obscurity than endure the fate that House Jorasco reserves for those who expose its darkest secrets.
Level 3: Diseased Hands
You learn to weaponize sickness with precision:
When you cast a cleric cantrip that inflicts damage, you can choose to deal poison or necrotic damage instead.
Channel Divinity: Affliction. As an action, you can target a number of creatures within 30 feet equal to your Wisdom modifier (minimum 1). Each target must make a Constitution saving throw against your spell save DC.
On a failure, the target gains the Poisoned condition for 1 minute. They may repeat the saving throw at the end of each of their turns, ending the effect on a success.
On a success, the target suffers no effect.
Level 3: Surgeon Domain Spells
Your connection to this divine domain ensures you always have certain spells ready. When you reach a Cleric level specified in the Surgeon Domain Spells table, you thereafter always have the listed spells prepared.
| Cleric Level | Prepared Spells |
|---|---|
| 3 | Inflict Wounds, Ray of Sickness, Blindness/Deafness, Ray of Enfeeblement |
| 5 | Bestow Curse, Stinking Cloud |
| 7 | Phantasmal Killer, Blight |
| 9 | Contagion, Cloudkill |
Level 6: Pesitlent Control
Your mastery of disease allows you to exploit contagion with lethal precision:
When you successfully target a creature with the Poisoned Condition with a Cleric Cantrip that does Poison or Necrotic Damage, the target is considered vulnerable to that damage.
Additionally, the target must make a Constitution saving throw against your spell save DC.
On a failure, the creature is Incapacitated until the end of its next turn as its body purges the contagion.
Whether the save succeeds or fails, the creature loses the Poisoned condition.
Level 17: Plague Mastery
You have perfected the art of controlling disease, allowing you to emanate a potent contagion:
As an action, you can emit a Contagion for 1 minute. While active, creatures of your choice that begin their turn within 30 feet of you must make a Constitution saving throw against your spell save DC.
On a failure, the target gains the Poisoned condition until the start of its next turn.
Creatures normally immune to the poisoned condition still make the save, but do so with Advantage. A failed save applies the Poisoned condition until the start of their next turn.
You can use this feature a number of times equal to your Wisdom modifier. All expended uses are recovered after a long rest.
