Rogue
Base Class: Rogue

Sawbones are cunning and unorthodox medics who ply their craft in the shadows. Often lacking formal training, they make up for it with intimate knowledge of anatomy, improvised surgical skills, and a willingness to experiment on the unwilling—or the unfortunate. Operating in filthy back alleys, secret basements, and dimly lit operating rooms, a Sawbones’ methods are crude and often painful, but undeniably effective. Whether mending allies’ wounds, exploiting enemies’ vulnerabilities, or crafting new ways to inflict harm, Sawbones use their medical expertise as a weapon just as deadly as any blade.

Back-Alley Physician

Beginning when you choose this archetype at 3rd level, you've gained extensive medical experience, though you lack any modicum of official education. You gain proficiency in the Wisdom (Medicine) skill. In addition, you can use the bonus action granted by your Cunning Action feature to stabilize a dying creature within 5 feet of you.

Surgical Strikes

Also at 3rd level, you have an impeccable knowledge of humanoid anatomy and know how to use that knowledge to your advantage. You score critical hits against humanoids on a 19-20. In addition, whenever you use your Sneak Attack feature, you can halve the number of Sneak Attack dice you roll to also force the target to make a Constitution saving throw against a DC of 8 + your proficiency bonus + your Wisdom modifier. On a failed save, you can cause it to suffer one of the following effects of your choice until the beginning of your next turn:

Cripple. The target's movement speed is reduced by a number of feet equal to five times your Wisdom modifier.

Daze. The target cannot take reactions.

Infect. The target is poisoned.

Maim. The target is deafened or cannot speak (your choice).

Expert Physician

At 9th level, you can swiftly mend wounds and cure ailments. You can use the bonus action granted by your Cunning Action feature to touch a creature and do one of the following:

  • Roll your Sneak Attack dice and cause it to regain a number of hit points equal to the result, provided you haven’t already used your Sneak Attack this turn.
  • Either cure a disease affecting it, or end one effect causing it to be poisoned, blinded or deafened.

You can use this feature a number of times equal to your proficiency bonus per long rest.

Cultivated Resilience

At 13th level, your experience working amongst blood and gore in dirty conditions has hardened your body. You gain the following benefits:

Improved Surgical Strikes

At 17th level, your anatomical knowledge has extended to stranger creatures. You score critical hits against any creatures, other than constructs, on a 19-20. In addition, whenever you use your Surgical Strikes feature, you can deal your full Sneak Attack damage in addition to applying an effect to the target.
