Base Class: Monk
The first monk to practice the Way of the Shattered Mind was a cultist of Cthulhu named Grey Metcalf. It was a dream where Cthulhu gave its bidding to Grey, and he single-handedly took a monastery to do his lord's bidding. Joining the Way of the Shattered Mind requires having already been driven insane by Cthulhu or one of the many tomes written by The Great Old Ones.
Projections of Madness
- Create a visage of Cthulhu, forcing all other creatures that can see it to make a Will save at -3. Failing it causes a loss in moral and a -1 to all rolls until the combat ends.

Reach into the mind of a creature with 5 Intelligence or less, forcing a Will save. If failed, the creature acts as if under the spell, Charm Person (if humanoid)/Dominate Beast (if target is bestial).

Cast Insect Plague at half range, and with only 2d10 damage.
Minor Necrosis
At 6th level, you gain the following abilities and traits:
- You may cast Blight, Chill Touch, Contagion, and False Life with no material components at the cost of 1 Ki points.
- Your begin to look gaunt as your insanity sets further, and the darkness of your mind manifests to heal you instead of taking Necrotic damage; the physical degradation, however, is not stopped.
- Undead up to half your level will react as if you yourself were undead.
Personal Abomination
At 11th level, Cthulhu deems you worthy of commanding an abomination of his creation, granted you can summon it. The first step is alignment. For a lawful creature, a chunk of lapis worth 200 gold is required; For a neutral creature, a diamond worth the same; and for chaotic, a ruby worth 200 gold. For a good creature, a sprig of wolfsbane is needed; for neutral, a glass eye; and for evil, an acid flask. Second, each CR has a unique material used for both creatures. For 1/4, a copper wire 1 ft. in length; for 1/2, a sickle; for 1, a human skull; for 2, a hobgoblin’s chestplate; for 3, two sets of artisan’s tools; for 4, an enchanted dagger; for 5, a spell scroll; and for 6, an enchanted longsword and one spell scroll.
Cthulhu's Chosen
At 17th level, you gain the following abilities and traits:
- You are immune to being charmed or feared. Additionally, when you would normally make a madness check, you instead gain 2d6 temporary Hit Points.
- You may spend 3 Ki points for a guaranteed infliction of charm, fear, or madness so long as the attack hits.
- You may spend 2 Ki points to cast Blindness/Deafness, Circle of Death, and Create Undead. Unlike with Minor Necrosis, however, materials are required for casting when applicable.
- Undead of your level or below now treat you as a lich, though you do not have their immortality or other powers.
