Base Class: Warlock
The Wretch is a patron of incredible misalignment and dismay. Your patron could take the form of an oversized bug or creature, a disgusting anomaly, or another entity of extreme filth and vile. You may serve your patron as a servant, soldier or worker, perhaps for reasons far from human.
Expanded Spell List
At 1st level, The Wretch lets you choose from an expanded list of spells when you learn a warlock spell. The listed spells are added to the warlock spell list for you.
1st Level: Fog Cloud, Ray of Sickness
2nd Level: Enlarge/Reduce, Web
3rd Level: Slow, Stinking Cloud
4th Level: Evard's Black Tentacles, Gravity Sinkhole
5th Level: Insect Plague, Contagion
Revolt
Starting at 1st level, you cause those around you in combat to feel severe discomfort and break their concentration. Creatures within 10 feet of you move as if they are in difficult terrain. Additionally, you can use your reaction to perform an especially disgusting act, forcing a creature within 20 feet to reroll as if they had disadvantage on the roll they just made.
You can use this feature a number of times equal to your proficiency bonus, and regain all expended uses on a long rest, or half of your expended uses rounded down on a short rest.
Toxic Touch
At 6th level, your patron's wretchedness begins to corrupt your body. You have advantage on Intimidation checks, and are immune to poison damage, diseases and chemicals.
Additionally, creatures that make contact with you quickly become infected with your patron's disease. When a creature makes a melee attack against you, they take 1d6 poison damage. Once you reach level 11, they take 2d6 poison damage instead, and at level 16, they take 3d6 poison damage. A creature that is grappling you once your turn begins also takes this damage. This damage cannot be deal to the same creature in the same turn twice.
Plaguebringer
At 10th level, your body is crawling with filth and disease. When you use your Revolt feature, the creature you target is also infected with a worsening plague, granting them one stage of plague. When that creature attempts to make a saving throw against one of your spells, or an attack roll against you, reduce it's roll by one for each stage of plague it currently has. The plague can only be removed via a curative or Remove Curse spell, a short or long rest, or by dying.
Baleful Blood
At 14th level, you have mastery over the plagues affecting your body. When a creature makes a melee attack against you, that creature gains one stage of plague. This cannot happen to the same creature in the same turn twice.
Additionally, you can manifest your plague into different forms whilst it afflicts your foes.
When a creature has three or more stages of plague, it gains disadvantage on saving throws and checks using one ability score of your choice.
When a creature has five or more stages of plague, you may afflict the creature with a disease of your choice, as if you had cast the Contagion spell.
When a creature has eight or more stages of plague, it gains one level of Exhaustion every 2 hours.
When a creature has twelve or more stages of plague, it must make a DC 15 Constitution saving throw. The creature gains a bonus on this saving throw equal to it's number of hitpoints divided by ten, rounded up. On a success, the creature is reduced to eleven stages of plague. On a failure, the creature dies.
