The spell needs to have the correct class (being mindful there's 2014 and 2024 versions of all classes except Artificer, so the spell needs to add the correct version or both) added into the "Available For Classes" box.
If this is public homebrew you need to click Add to Collection - you can only do this if your have a Hero or Master subscription. This will not be necessary if the spell was made by you or by anyone in the same campaign as you.
Ensure Homebrew is enabled in the Home tab of the Character Builder.
Ensure your character is of the appropriate class and level to have the spell appear in their available spells to learn/prepare.
The spell will appear as normal.

If you want to add the spell outside of normal learning/preparing, due to the DM granting it to you as a special boon, then that gets more complicated. If the spell is a "one a day" type of thing, it can be added via a homebrew feat. But if this is a "can cast through spell slots" deal you need to make a homebrew copy of the subclass or species you are using. If you provide the full details of either scenario I can show you how to do it.
