Check box if the weapon is your Hex weapon. Check Pact if the weapon is your pact weapon. Check both if the weapon is both.
The weapon granted by Hexblade does not have to be the same one as your Pact Weapon. Check the box that applies to add that feature's bonuses to the weapon. If the weapon is both, check both boxes to add relevant bonuses from both features to weapon.

do you select both tabs or does the hex tab add the bonuses for the hex feature for the class?
