Proficiency is added in both ways, either your (lucky d20 roll) or your oponents (unLUCKY d20 roll). You only choose the roll, proficiency is added anyway. Thing about having advantage/disadvantage : If you roll third d20, you get to choose between 2 lowest (if having disadvantage) or 2 highest (if having advantage). :)
The part about the advantage/disadvantage and only choosing from the two lowest/highest is incorrect as per the official Sage Advice from 5/18/2015
How does the Lucky feat interact with advantage and disadvantage?
The Lucky feat lets you spend a luck point; roll an extra d20 for an attack roll, ability check, or saving throw; and then choose which d20 to use. This is true no matter how many d20s are in the mix. For example, if you have disadvantage on your attack roll, you could spend a luck point, roll a third d20, and then decide which of the three dice to use. You still have disadvantage, since the feat doesn’t say it gets rid of it, but you do get to pick the die. [Source]
EDIT: Additionally, your proficiency should only be added if it was added on the initial roll. For example, if you are rolling for Athletics with disadvantage, and you are not trained in it you would roll 3 d20's, choose one, then add your STR mod to it without proficiency.
In effect, you roll the additional die, and choose which to keep, before any modifiers.
Then you apply the modifiers relevant to the roll - relevant ability bonus, proficiency bonus if it applies, and all other modifiers (such as an equipment or spell-granted bonus).
Tip: Remember, a Death Saving Throw is a Saving Throw (and thus Lucky is applicably to it), even if it doesn't have an associated ability.
