1.) Every damage roll. Hexblade's Curse, Hex, and Agonizing Blast all stack on each of the d10s that hit. A tuned blast-lock can be really mean, and they can do it all day long from safe range.
2.) No, because weapon and Sneak Attack are a bunch of dice stacked on the same damage roll. I don't remember how Green-Flame Blade works and don't have SCAG, but your damage roll for a successful sneak attack with a rapier and Hexblade's Curse is d8 + 3d6 + DEX + Proficiency (for example). It's all one "damage roll" with a bunch of dice.
Some classes (like Rogue) are tuned for single big whallop attacks, while others (like Warlock) are more tuned to doing more damage with multiple strikes.

1) Applies to each "damage roll" - thus the bonus applies to each Eldritch Blast when you are throwing multiples
2) What about if my Swashbuckler/Warlock hits with Sneak Attack and GFB....does the bonus apply to the Weapon, GFB, and the Sneak Attack?
If it's only item one at some point I may be sheathing the pact blade and start blasting away!
Not sure I know what gfb is, but from RAW it would trigger seperately off the weapon and the sneak attack, as they're seperate damage rolls.
