I built a hill dwarf barbarian, average HP build.
He's got 49 HP at level 4.
I added the toughness feat (+2 hp per level) his HP stayed at 49 rather than increasing by 8 to 57.

UPDATE: I switched to the character sheet view, then back to levels, now it's 57...
