I selected musician as my origin feat and fiddle is an option but the only way to add one is to make it yourself. I think if the player can choose fiddle as a proficiency they should be able to get one in the list of equipment or am I making some insane mistake?

The Fiddle option for instrument proficiency is drawn from the Lord of the Rings RPG. I'm guessing that it won't let you add items from that book to your inventory unless you actually own it, but the proficiency list isn't restricted that way.
Is the "viol" option in your list? It's essentially the same thing.
I looked at that, but at some level you could say the same about a double bass, it just felt too far. I still think throwing it in the equipment makes sense, but that could just be me.
