Currently, taking the simic scientist background allows a character to take an additional proficiency, as well as granting them the proficiency in medicine.
please can you check whether your character already gained the Medicine proficiency from their class, prior to selecting background?
When a background specifies a particular skill is gained, but the character already selected that skill from their race or class, the background (as per the rules) allows a different skill to be selected.
I can confirm that simic scientist is giving proficiencies in arcana and medicine in addition to two skills and two languages of your choice.
[Edit] Additionally, the azorius functionary background does not show which skill proficiencies it gives (insight and intimidation) in the builder. It does properly give them though.
