Oh, Grim Hollow and Drakkenheim could be set whenever, just like Ghosts of Saltmarsh. So, those ones don't count. However some adventures like Out of The Abyss and Tyranny of Dragons are set around a specific timeframe.
I was thinking that maybe Tomb of Annihilation could have Kongamato, Mbielu Dinosaurs, Spinosauruses and J'ba Fofi (from Tome of Beasts 1) added to the jungle encounters while Princes of the Apocalypse could have Elemental Evil Giants (from Bigby Presents: Glory of the Giants) added to make it more difficult. Maybe I could also adapt Abishai and Dragonspawn of Tiamat to 5e for the Tyranny of Dragons campaigns.
I think you might want to try breaking it down into how many sessions it would take to complete each of those adventures. A considerable number of them will take approximately 50 sessions of about 6 hours per session to complete. Let's say that it averages out to 30 sessions per adventure, you have 49 adventures that's 1,380 sessions and 8,280 hours. Let's assume that you can squeeze in 4 sessions per month; that's 345 months, 12 months per year it will take you approximately 28 years to complete that list.
I'm not saying it's impossible; this guy did it (and is still doing it), but it seems unlikely.
I don't think the time estimate is accurate. 300 hours for a published adventure seems really high. a level 1-5 adventure typically takes around 20-40 hours, so even a campaign-length adventure, like Shadow of the Dragon Queen, shouldn't take more than 100-150. It's still unreasonable to string all of them together, plus it probably wouldn't be very fun. It would end up being really disconnected. Lost Mine of Phandelver and Dragon of Icespire peak are designed for low levels, so they have small-scale stories, and they wouldn't be interesting past level 5.
When we did RotFM, it took a bit over two years for 13 levels, a session every two weeks at 2.5 hours apiece. That's about 10 hours a level.
I could see 300 hours for a full L1-20 since later levels take longer. I'd say most published adventures are in the 100hrs mark.

I ran a campaign that included some of each official module up through Descent to Avernus (the newest one at that time) and it took my players to level 17 over three years.
In Session Based Advancement the DMG suggests between 28 and 46 sessions to reach level 20. I've rarely seen games actually progress that fast, if you actually count xp, after level 3 it typically takes 12 routine fights or 6 tough fights to advance a level, either of which is around twelve hours of play time unless everyone is really on their toes. Double that for doing things other than fighting, and you're looking at 400+ hours to level 20 (Critical Role campaigns were generally 500+). Games that use waypoints or story-based progression are generally faster (my Curse of Strahd game went level 1-10 in around 120 hours).

As for how they get to other settings it's either through Ravenloft or Spelljammer.
