For Jack, not all tentacles are the same. These are not aquatic (like a squid or an octopus). These are "dry". Thicker than your thigh, covered with a mix of spines, hair, and scales.

The glaive that Isk has been using to help steady himself begins weeping a deep crimson mist at the appearance of the tentacle creature. Isk scans the room quickly and then turns to the door that is blocking their way out. He points a finger at it and an intensely resonant BOOM, like a giant bell, sounds shaking the door (Shatter9 dmg to the door).
Isk looks back to his companions, and channeling his inner Corporal Hicks says, "Friends, we are leaving!"
"Not sure if this will be relevant right now or not," Froskan says to the others now that they know "Theo" isn't actually Theo, "but I realized something a minute ago. How were we able to see the Skulks that chased us back to the portal? They weren't invisible..."
(Does Isk's spell break the door open? Might affect what I do for my turn)
“You didn’t? I thought you did! Now, come on guys, we have to go!”
If door is now broken open: Jack runs out the door away from the tentacled monster, and will take his time to Defend Himself. “Come on!”
If door isn’t open, and he can’t get through: Jack runs close to the door, realizing he can’t get through. “No, no, no...” He grips his drum, and shouts at the monster. “Hey! Ye wanna year a song!?”

Jack is pumped up, as he can sense the spell take effect. But then he is slack-jawed, as the creature shrugs off the spell (legendary resistance) and continues moving forward, as big as a giant. (Image as it starts to enter the light of your room: link )
Iskander uses Shatter to cause the doorway and part of the wall to collapse in front of the creature. It is delayed long enough for the party to pull Trim and Reverie towards the exit outside of the temple. As everyone rushes through the exit, you can see the toothy maw crushing a boulder in its path to pebbles.
The party finds itself standing on a flat rock in the Shadowfell. This is the same rock that you were on when you thought you escaped the Shadowfell earlier (post 3036 link ). There is no temple. No skulks. The flat rock doesn't have the runes you saw before.
A hundred yards in front appears to be the flat rock that you were trying to get to, where you first arrived in the Shadowfell. Something must have obscured it from sight before now.
Behind you, you hear boulders being crushed. The creature is still there. It is most of the way out an underground passage, being slowed by the rockfall from the Shatter spell.

Froskan follows, but asks, "Do we think it's the real thing this time?" Just in case, and since they're close to the supposed exit, he opens the shutter on his lamp to reveal anything that might be hidden around them.
The party rushes to the other rock slab. It is covered with the expected runes of teleportation, planar gates, and warding.
Reverie pulls out the crystal, and hearing the roar of the creature getting closer & louder, says "Thereisnoplacelikehome thereisnoplacelikehome thereisnoplacelikehome" in a single breath. The crystal responds promptly, and a dark smoke archway forms (like it did in the Oghma temple back in post 2850 ). Before the creature's tentacles can grasp you, the party enters the planar gate, which closes behind you.
After the brief psychedelic light show, you are back in the temple of Oghma (...you hope).
There are 3 clerics nearby, in Oghma-style clothing. They look at your group warily. "Stay where you are. We need to look over your group, to make sure that you are who you appear to be. We do not want any unexpected visitors from the Shadows."
(Froskan has the Lamp of Revealing open, so you are confident that you were not accompanied by anything invisible.)

Reverie catches her breath as she holds up her berserker scimitar, defending herself and holding the crystal in her hand firmly. ”Where is Theo!” She speaks loudly of the high priest’s name before she calmly walks forward to the three clerics, showing the crystal, ”Where is he? If you know where he is, tell him that we are here and one of them is holding this crystal he gave to us. We need to talk with your high priest immediately, must report to him about what we just went through BEFORE you could look over us.”

The cleric who spoke earlier gives a stink eye to Trim, Isk, and Reverie in turn. "Bishop Theo will be here shortly. He gave an order to be informed when your group returned. But he will not come in, until we are certain you are not a danger."
The cleric next to him activates a gemstone, and wears it like a monocle. She stays out of arm reach, and scrutinizes the party. Froskan recognizes it as a Gem of Seeing.

The party was gone almost 6 hours. Bishop Theo wasn't going to sit around this room waiting for your return. It is not surprising that he would be working in other rooms of the temple.
As far as you can tell, things appear to be on the level. There weren't discrepancies like last time. (For instance, the return crystal not activating on the first try. You are free to look at the earlier "spoilers" for the other party members.)
When the monocle cleric looks over Jack, she stops and polishes the gem, like it is smudged or something. She doesn't seem entirely happy about Jack, but she still gives a nod to the other cleric. After a moment, Theo walks in, and looks concerned at your group.
"You told my aides that there was trouble in the Shadowfell... Did you get the book to the Sages?"
