Hey is it just me, or are you not able to select proficiency for Surgeon's Tools at all? No background, class, or feat (even the ones that come with this book)
Any fix for this because I have a Doctor Cleric that this would be perfect for.

Seems like it's an issue in the character builder specifically. You can add it directly on the character sheet by clicking inside Proficiencies & Training, then using the sidebar.

I mentioned it to the devs the other day as part of a short list of 2024 issues that still get mentioned the most, but I don't know what they're currently working on or what their next priorities are.
The other ones I'd mentioned were Ritual Caster, Martial Arts Die, Warlock invocations, and (now fixed!) XGTE feat prerequisites.

You can manually do it by clicking your tool prof box on your character sheet and adding a custom tool, not a perfect fix but it's something

The Daydream spell from this book can be selected for the Fey Touched feat from the 2024 PHB, but not the one from Tasha's Cauldron of Everything. Additionally, Corrupting Ichor can be selected for 2024 Shadow Touched, but not the 2014 version.
