Particularly heinous crimes leave psychic residue that causes a place to become sentient. The urban legend could be any sort of haunted house story.
A grey jester (a 3e monster) is a fey spirit that drains the emotional capacity of nearby creatures, feeding off of intense emotions.
The clock man. Its face is a clock. It feeds on youth and can only be destroyed by an elderly person. It has a particular hatred for creatures that are chronically late for appointments.
A local nature spirit hates urbanites. It causes its targets to overflow with vitality (read: temporary hp). When the temp hp exceeds the creature's hit point maximum, the victim explodes.
An ancient swordsman possessed a sword that could cut through flesh, steel, space, and time. When the swordsman cut through reality, they were... changed.
The city was constructed over the prison of an ancient entity of plagues, which awakens every 74 years and creates a new pathogen.

Well, high fantasy world leads to thoughts becoming real and the monster now exists, a construct made up of knives, axes, swords, scissors and whatever the people left lying around. It can see through candles, so the lair is very secure. I wanted to make a pop-up-book for my group back when I had this idea but I never got around to it ^^"

I made a thing for a campaign that never came to fruition called the slumberthing. its (a dreams druid who got trapped on the edges of the dream world and changed into a fey) the monster under the bed. it steals childern's eyes to complete a grisly ritual to contact the DM and get them to lift its curse

There's the Bodach. An old boogeyman who has long limbs, a skinny body, and no face. There is a Dnd statblock for it.
The Scissorman is a Boogeyman with scissors for fingers. He cuts the fingers off of naughty children.
Spring Heeled Jack terrorized London for a while, breathing fire, attacking women, and leaping across the rooftops.
There is a Shadow Man who haunts a theater. He approaches people, sticks his hand inside of their stomach, and squeezes their organs to cause immense pain.
El Silbon is a whistling boogeyman who whistles as he walks through the streets and has a bag of bones slung over his shoulder.
