My players were playing a one-shot level 20 campaign were they were gonna kill Cthulhu. Unfortunately, the players were had the big dumb that day. To cut a very long story short, he took over the multiverse. He enslaved all other creatures and let aberrations rule every plane of existence with an iron fist/claw/tentacle/whatever preferred appendage they have. I expected the players to get mad, but they actually were really happy. They want me to make this a setting for an actual campaign in this dystopian world. I am totally on board with it! But I want some ideas on how to run it. Thanks for the help!
I only have a passing knowledge of Cthulhu mythos but isn't there a cult that worships Cthulhu? If so, I'd have them become the primary religious factions in the multiverse. Have the original religions become decimated under the weight of the new cults, and trying to keep the followers they have left safe. You could even have the original gods themselves actively leading their religions on the material plane.
Also, how do the other GOOs feel about Cthulhu's new power? Happy they're more minds to corrupt? Angry that Cthulhu's cult is more powerful?
Finally, I'd imagine that you'd need to find/create a homebrew sanity system that your players need to use. I have no suggestions on this front on where to look but its something to think about. Just don't lose you mind finding it.

I agree with number 2, but that doesn’t mean the other Old Ones wouldn’t be fighting each other over the souls of their worshippers (or of warlocks willing to make a pact with them).
Also none of the GOOs really has a gender as humanoid species understand the concept.

Also, keep in mind that not all Warlocks agree 100% or even 51% with the agenda of their patron. There definitely would be some that try to subvert or distort the will of Cthulu if Cthulu suddenly went from being a mere background presence to dominating every world everywhere.
