One of the brain puppies rushes forward and attacks Xap. Xap, you can take an attack of opportunity as it moves into yur range. It will also take 15 cold damage if it lands its attack... Claws. Melee Weapon Attack: 16 to hit, reach 5 ft., one target. Hit: 7 slashing damage. Devour Intellect vs Xap. The intellect devourer targets one creature it can see within 10 feet of it that has a brain. The target must succeed on a DC 12 Intelligence saving throw against this magic or take 9 psychic damage. Also on a failure, roll 3d6: (10) If the total equals or exceeds the target’s Intelligence score, that score is reduced to 0. The target is stunned until it regains at least one point of Intelligence.
Xap
You just entered a chamber filled with small dog-sized brain creatures. One of those just rushed forward and attacked you. Depending on your opportunity attack it might not be alive any longer, so please account for that possibility in your action. It is your turn to act.
This post has potentially manipulated dice roll results.
As the brain puppy charges to jump on Xap, he instinctively raises his pact halberd and braces for impact.
PAM Opportunity Attack: 25 Damage: 10
The puppy manages to get past the halberd after taking some damage and it rakes its claws into Xap. The damage is absorbed by a blue shield like energy that surrounds Xap’s body and a flash of retributive energy surges back into the creature. It crumples to the floor, dead.
Xap will move to D8 and begin attacking IN10 from 10’ away, stepping forward to E8 (F8) as needed to continue his PAM assault with the pact halberd.
I’m guessing the armor of Agathys did not go off, so the above attacks would have gone into IN7 first and Xap would have moved down only after IN 7 was dispatched. See below.

Xap
(No need for the devour intellect save, the brain puppy died before it could use it. Save your blessed by the gods ability.) Xap wounds the intellect devourer as it moves in, then sees it fall to the cold that covers his armor and gear. He then moves and strikes down another of the brain-things with three well-placed strikes of his weapon.

Moves forward and attacks Karen. it sends its mental attack at Hoghead though... Claws Attack: 21 Damage: 5 Devour Intellect vs Hoghead. The intellect devourer targets one creature it can see within 10 feet of it that has a brain. The target must succeed on a DC 12 Intelligence saving throw against this magic or take 3 psychic damage. Also on a failure, roll 3d6: (14) If the total equals or exceeds the target’s Intelligence score, that score is reduced to 0. The target is stunned until it regains at least one point of Intelligence.
Will wait for Hoghead's save, as that might change actions for the next enemy actions...

karen and Zenga (blessed)
Karen has an Intellect Devourer right ahead of her, and a second to her NW. Zenga is not currently in melee with any of the brain puppies, but there are plenty of targets he can pick between. It is Karen and Zenga's turns to act.
Zenga moves to G7 and attacks IN9 and moves counterclockwise to next target if he happens to drop one. With his short sword out he takes two attacks with that before spending the ki to then do Flurry of Blows.
Shortsword Attack: 12 Damage: Unable to parse dice roll.
Karen strikes at the brain puppy near her, her weapon hitting the small creature, but doing less damage than would be expected.
Zenga (blessed)
Zenga moves to attack the furthest southern intellect devourer, and hits it three out of four times, dealing damage. Again though the amount of damage does not seem to be what would be normally expected.

Tydir calls out in rage and frustration as the horrid monsters close in on his comrades 'Back you fell creatures, back!' a glowing, ghostly hammer shoots out from his physical weapon and spins toward the monstrous form attacking Karen, even as he does, he unships the crossbow from it's home on his back and squeezes off a shot towards the dog sized creature.
