As the shock of his sudden injury consumes him, Sam perceives Toril as if he were sta ding on the Sun, watching the blue ball drift through space. Moving closer to the spinning globe, he sees more detail and from the pattern if mountains, peninsulas and larger land masses, a writhing mass emerges, encircling the entire planet. The world serpent lives and breathes throughout the whole of Toril. As Sam feels himself draw nearer and nearer to the surface of the verdant peninsula beneath him, he finds the eye of the serpent staring back at him.
(I just realized that if Sam was within 10 feet of Angus, he should get a +3 to his saving throw....)
There's no mechanical effect here. Unless he loses the other one. Let's just go with it so something interesting happens. It's little more than roleplay, but if you want to reverse it, the condition follows the injury rules in the DMG.
If I wanted to dodge it I could have just said the world serpent was not on this plane and Sam would receive no vision and pay no price.

Angus will lay hands on the lizardman to heal the damage, but of course, he can't make the eye grow back.

<My impression is that everything is on the ground floor of this tower and tbe silver light seen through the window is from the large ocular centerpiece, situated on the ground floor (i.e. the only floor). The eye above the window is just a carved symbol to identify the tower's affiliation with the Divination school of magic. I was quoting before but the description is not very clear.>

Sam being from Chult, might actually know the place by geography alone. Most people haven't seen the interior of the peninsula, but Sam certianly has. I don't think he's seen this exact place. I'm sure not during the events I've witnessed, but he may have seen it at some point. It may when be the site of thw inciting incidents on his path to adventure. I'll leave that that @Legion4. But, yeah, it's in Chult.

Yeah, the author guesses you might use the Divination ball here to ask where the missing piece is. Now that you're aware of the risk, I don't think it's a problem to spoil that riddle, as it presents its own dilemma. You can ask and you'll find it, but it might cost you an eye. Technically, you can ask as many questions as you want*, as long as the one asking still has at least one eye.
*by questions, I mean "location of x" inquiries.
Failing that, you might just proceed to the nearby Wellspring of Divination. It's optional, but for each minute spent meditating on a nearby bench, you can learn another bit of lore about the city.
Observatory: This conical tower is held relatively upright by a ring of arched buttresses. Expressionless humanoid figures stand in silence around it. The roof has a large, ragged hole in it where something large smashed through.
If you should apprach the observatory, you will notice Three galvan magen, three demos magen, and one hypnos magen stand guard outside the observatory with orders to prevent strangers from entering it. The hypnos magen telepathically orders intruders to leave the area.
1. I don't think I declared that magen are the guys you met in the lightning ball (or whatever it's called) arena. They're magical constructs that appear humanoid with greenish blue (blueish green?) 'skin'.
2. The depressed area seems to be what is left of a once thriving market district. The collapsed tower is at the end of it.
3. The observatory itself is at the top of the image, unfortunately I cropped the top if it off but you can see it in the full city map right by the number '8'. Unfortunately a red line is drawn through the tower. Not much special about it at this distance, but as mentioned in the description, there is a big hole in the roof.

“ well Angus wait here for two seconds” for the idea but will shake it away he turns back to the orb of divination, then turns it away from him so his left eye would be looking at the group then will pick it back up and where the missing eye would be he will attempt to put the orb into his eye socket and use that as his left eye
I actually really want this to work, but for really terrible reasons. Sam is magically enlarged at the moment and that effect will almost certainly end suddenly, which would cause Sam to reduce to Large size while the nigh indestructible artifact lodged in his skull remains 3ft in diameter, exploding Sam's head.
However, as I am a kind DM, I will not allow this to happen to Sam.
The following is also given in the source material:
"The orb is magically anchored to this chamber and cannot be removed or destroyed. The only limit to how many times it can be used is how many eyeballs the party has remaining."
From where you stand you're a stones throw away from the observatory, the open air market, the enclosed structure thst was also a market, basically a mall, and the fallen tower of Necromancy.
I'm just going to tell you that the missing inscription from the tower of Divination is in the observatory. So eons already paying the toll for the info, may as well have it.
Angus whispers to his companions, "We need the information in that observatory. I'm not sure how powerful these creatures are, but a nice fireball might be a good way to start things off here..."

To access it you would walk down into the slightly lowered area, passing by the observatory.
