The second time, the rogue gets two hits but the first time when he attacked Skiirod, he should get 1 attack following the rules. Skiirod still goes down though because crit and the second attack misses regardless (and it did a total of 21 damage in his second round.
Skiirod get an extra spell slot.
As for why the rogue gets two attacks, he is a level 2 rogue with two light weapons. He can attack with one and use a bonus action to attack again.
That is fine i was just very confused, I understand the rogue just that turn threw me off completely so i was just going with whatever. all good.
Looking at it with hindsight, I don't think there was actually anything Skiirod could've done to prevent going down. The dice were that against you. Dragon breath would've probably effected your allies so that's not good. Your attack roll was low so you would miss. The rogue did so much damage, not even Lay on Hands could have saved you.
It's always the way. One of my encounters was a pair of quicklings running around doing pranks. It took them forever to actually catch one of them (given there's six of them). They got a nat 20 on a grapple check, but then so did the quickling and it had a better modifier.
As mentioned, it is well packed away. It would take way too long to rummage for and get out, light, then throw. Too impractical in her eyes. Also she's definitely not one to do friendly-fire.

I know but I could have moved and risked the AoO lol hey it's fine, I have hit maybe 3 times over all the battles, so yeah not doing much anyway.

I guess the party were wishing they'd left it as-is. Trading Elsara's secret bomb seems like a much less deadly encounter :-p
At least Elsara still has her bomb. Maybe it would have been a good idea to throw the bomb at Gym and try to take out 4 guys at once.
