Val slips into the cave with no issue. It is dark inside. He can still see up to 60ft thanks to dark vision.
The first thing Val notices is that the cave walls are surprisingly straight. The walls don't curve in or out like how its common with most caves. This gives the cave a more box-like shape that is common in man-made rooms rather than a cylindrical tunnel like shape that is common in most natural caves. They are also just barely smoother than any cave wall naturally should be. These two features give the cave an uncanny feeling. The path leading to the entrance is 10ft wide and stretches north for 60ft and then turns east sharply. Moving forward to the north wall and turning to the east, Val sees the tunnel stretch eastward into the darkness. The tunnel is longer than the range of his elven darkvision.
The tunnel is quiet this time around. Elsara informs you that the cave has been warped using transmutation magic. This explains the almost man-made appearance. You finally find some stairs at the end of the 120ft long hall. Descending the stairs which bends north then east then north then east again, you come to a room that is 30ft on each side. On the south the room extends further down to a section that's 20ft wide and 60ft long. To the north of the room is another set of stairs leading up. To the south are three sets of doors. To the east is a large metal door. This place is also dark.
The more Devarus saw and heard, the more ill at ease the aged guard appeared to become. Yet, he followed after Runt at the very least, eyes ever on a swivel for danger. "Transmuation? Mutated Orcs... Anyone else getting the feeling something is at play here than just a bad case of food poisoning." He quietly and halfheartedly jests before his eyes settle on the metal door to the east.
"What is your feel, Runt? Common sense says the three to the south are a, uh, bunker of sorts. Meaning might luck out with a few sleepers. But that one--"He gestures past her to the metal door. "Nothing but trouble. Could lead to the source of the strangeness that's been going on though."While she deliberated (or not?) Devarus trains scattershot towards the southern passageways.

The party heads up the stairs. The stairs are steep and move in a quarter circle. As you ascend further and further, a smell of vomit, blood, flesh and fecal matter get stronger. Finally you reach a large room. No artificial light sources but the room is dimly lit by sunlight coming from the windows to the south. This room would have been pitch black had you entered at night. There are three large pillars in the room, all made of stone. The floor is strewn with half eaten bodies of orcs and villagers alike. Blood, vomit paint the floor. To the north is a large door-less gateway that leads to darkness (too far away for you to see what's there with Dark Vision)
However the most horrifying thing in the room is a large morbidly obese orc. You wouldn't have been able to tell it was an orc if it wasn't for the face. It is almost 8 feet tall and equally fat. It's legs its legs are like short tree stumps while the arms are like long fleshy snakes. The toes have retracted into the feet, giving the legs a elephant like appearance. You can see the fat and tumors in the arms and fingers shake like jelly as the orc bites the head off an orc corpse and begins eating it, chunks of flesh and drool coming out of its mouth as it chews. The body has lost most of its orc-like shape and gained a more blobish appearance. Hard tumors and bits of calloused skin remain isolated on the sea of fat that is the orc's body. As soon as Elsara sees the creature, she grabs her head and quickly dismisses the Detect Magic. She informs you that Affectus Uti is practically oozing off this monstrosity. It sees you and lets out a echo-y groan. You her chattering and breathing coming from the darkness.

White space is just there to make the make look less cluttered. Floor (-0); Stairs(|); Pillar (O) The underline indicates that there is a height difference between the floor and the stair. This is not something to worry about unless you want to jump from the floor down on to the stairs. It will call a DC10 Acrobatics check. Failure means you will lose some movement speed. Nat1 means you got unlucky and fall down the stairs. That will hurt.

Elsara's blast, while it did nothing to the creature, snaps the aged half-drow from his funk. "U-uh! Look alive, people. Sounds like mores a coming too."
Devarus rushes 30ft forward and fires a shot from the great crossbow already in hand. Then...
"Now, Runt! Move in." He commands soon as the shot finds purchase among the flubber; chi becoming visible at the same time as Runt. Whereas first they were of different colors, Runt's 'aura' matches Devarus own purple tone before fading into a lingering cloak. "And do not waver, young one. We stand as one."He intones, half-starting to believe his own words as the cloak of Chi about Runt grows in visibility. So much so, in fact, he drops scattershot to take out his longbow and fire upon the creature in a surge of adrenaline. If it should land, the chi laden behind the shot flares and digs even deeper as Devarus started saying curses in goblin at the oversized not-Orc.
(Using Maneuver Attack, inflicting an additional 6 non-magical piercing damage. Runt can spend her reaction immediately to move 1/2 speed closer to the target.)

The Glob groans loudly as it gets peppered with many small darts from the scattershot (Extra rolls for large size: 23 Dmg: 7). Then the arrow flies through and sticks into the large rolls of fat (WIS: 12). The creature groans angrily at Deravus. The bulky body of the creature has mostly caught the arrow (Reaction: Reduce damage taken from Longbow to half: 6)
DM: Runt, thanks to Deravus, you can move up to the blob if you use your reaction. You will be within melee range and have both your action and bonus actions left. Do you still want to dodge or would you like to Rage and Attack?

Runt runs forward holding her big hammer in front of her to guard against incoming blows while Val draws his bow and fires a shot while running west. The arrow flies forward but misses the blobs shriveled head by a foot.
