If you take the "Ready" action to trigger an attack as soon as you are attacked, does this count as an actual "Attack" action, meaning you can then bonus action with your polearm feat for the 1d4 damage? Or is it treated more like a reaction and therefore the qualifications of using your polearm (only after an attack action) would not justified?
2nd question. In cases where a creature suddenly appears within your reach (teleporting, or they were invisible and attack you and now become visible) does this count as an opportunity attack situation because they have appeared within your reach? Or is it only when you can visibly see them move to within your reach?
Readying an attack doesn't trigger Extra attack or bonus action as these can only be taken on your turn.
Teleporting never provoke Opportubity Attack. Becoming visible also doesn't count as entering your reach.
well that just nerfed my polearm quite a bit lol just kidding thanks for the info! I figured the Ready wouldn't allow the polearm extra attack but wasn't sure about the becoming visible within your reach part but that makes sense.
Using your reaction to take a readied action is treated as using your reaction.
Teleporting and becoming visible are not movement. And opportunity attacks can only be made against creatures you can see, so it didn't trigger when the invisible creature actually did move (unless you can see invisible creatures).

Entering your reach refers to them specifically moving into it
