The first one. T>A>T>A, plus an additional T>A if you attacked more than one target with the first two attacks. You don't need to use all the teleports if you don't want to obviously.

Yes, you can. The description says "If you attack at least two different creatures with the action", not "if you hit".

I highly suggest so. While it does say a third creature, the wording doesn't imply that this third creature has to be a different one.

It's like the hunter ranger's 11th level (volley) ability with the added teleport part. Three separate creatures are needed.
