To be honest, I was about to post in the adventure thread but this post seemed quite intriguing.
The rule about using the same hand for holding a focus and making gestures can be found in the PHB, under the Spellcasting chapter.
I would rule that a shield either carried or wielded in a hand occupies it such that you can't make weapon attacks or complete S components with that hand unless you are using that shield as the focus. If wielding a shield didn't require an occupied hand, the text of the war caster feat wouldn't make sense. Carrying a second shield would obviously occupy a hand -- unless you are talking about strapping it to your back.
A spell focus needs to be hand held for all classes, simply because they do not only substitute for unpriced material components but also the same hand can be used for somatic components as well.
War Caster feat is there for mostly non clerics, as through it a wizard can cast spells with verbal+somatic spells without using their focus.
Divinely Favored feat, if your DM allows it can be used in similar or even better ways too. Eg. a wizard/cleric of magery deity fashioning his/her shield with holy symbol and also using it as a spell focus for int based spells (if he/she chooses int from the feat). Thus he/she can effectively cast all spells with both hands full as long as there is no material component with a given price involved.(both cleric and wizard spells). My current character is a Deneir Priest/Order of Scribes wizard (while Deneir holy symbol is candle on an eye, he also has another, open book illumunating its surrounding, planning to use that holy symbol on a shield and making it spell focii if DM allows the usage of feat in Faerûn, it is not like i am joining melee but this way i can wield spell wands, or a sling, can hear people saying why not use cantrips instead of small pellets, my character does not like over use of divine and arcane energies and believes it might have bad side effects for the realm in the long run, he is more like a bookworm, a walking encylopedia you would want around) aiming for this...
Anyway since the hand holding the arcane focus can also be used for somatic components via above feat shield can be used for somatic components(it is still a hand hold object) Mask is another issue, if a cleric (of Mask perhaps?) asked me to make his/her holy symbol his mask, i would require him to hold his mask for spells that require somatic components, and probably won't allow wizards... You may think of Rashemen witches when talking about masks, but i doubt they are using their masks as spell foci...
