I have a player who decided their Tiefling wanted to fashion a bladed tip for her tail, I allowed it with a "yea but".
Technically a prehensile tail is not something a Tiefling is gifted with, however using down time to learn how to do certain actions with the blade tip and movement could allow them to create a whip like attack with it.
Letting the tail be prehensile from the get go allows a bit more mobility for the character which, depending on the skill of the DM, may cause some unexpected results in combat and other skills. However, I don't see anything game breaking with it and would tie it to a Strength (Athletics) or Dexterity (Acrobatics) check for successful attempts.
It depends on what you want to allow them to do with it. If you let it be used like Mage Hand then it could replace Thaumaturgy as a racial trait. If you want to be able to hang from it, pick locks, etc. then it would require a feat or replacing Infernal Legacy completely. In any case I would not allow it to wield weapons or shields as that breaks too many things.

I have a player that wants to be a tiefling but have a prehensile tail. is it possible to do that from lvl 1? or should it be a feat they gain?

I'd let them have that but lose a different racial trait to make up for it.
