As DxJxC linked above conditions are outside of a creatures(or players) statblock and are this not statistics and as such would not be impacted. But hey, your table your rules.
You can't polymorph into PCs - only into things in the monster manual. For example, an evoker, or a warlock, or an adult bronze dragon.
Another point is that the polymorph has a duration. It might say "forever" but what that really means is "forever until dispelled". And when it is dispelled the original PC is back, along with everything that was affecting them at the time.
Polymorph doesn't remove anything - just puts it on pause.
