Where in the rules dose it say that Instant Fortress dose not open in the activators face the moment the puts it on the ground and say the word? I see it as you HAVE to put it on the ground and say the word intently or it wont work.
The rules don't say what doesn't happen, they tell you what's allowed. Nowhere does it say an Instant Fortress immediately expands when placed on the ground, so it doesn't.
The rules for Instant Fortress say "You can use an action to place this 1-inch metal cube on the ground and speak its command word", implying that an action is required to activate the item (which seems unlikely to do accidentally... especially considering how much damage it does). Also note: even if this were done accidentally, the activator would not be in the damage area since the description says "When activated, the tower has a small door on the side facing you".
If there is no specific ruling then everyone in the area is hit "Each creature in the area where the fortress appears must make a DC 15 Dexterity" means everyone. Can you move before it happens? but the dex save is about moving out of its way, as you move out or run out of it. No were dose it say it expands on the end of your turn.

Why would you count the user as being inside the area that the tower expands into? No where does it say that it expands to fill a radius around the cube. I have always played it that the door appears where the cube is place and the tower expands into the area behind it.

No: the Tower appears adjacent to you (not on you) when created. The door is on the side facing you. You can open it (with a bonus action) whenever you wish.
The way I have had it explained to me is the the fortress expands from the door outward. Basically it grows in front with the door facing you pushing everything out of its way. Hence how the wording is placed.
Each creature in the area where the fortress appears must make a DC 15 Dexterity saving throw, taking 10d10 bludgeoning damage on a failed save, or half as much damage on a successful one. In either case, the creature is pushed to an unoccupied space outside but next to the fortress.Objects in the area that aren’t being worn or carried take this damage and are pushed automatically.

First of all: Instant fortress. These discussions become much easier when someone (preferably the OP) links to the subject.
And yeah, it doesn't say how it expands, but it is obviously not supposed to flatten its owner, so it must expand adjacent to them.
