According to the many resources I have read the Raven Queen and all affiliated to her are not really known to anyone other than those who delve deep into arcanic studies.
My questions are:
When playing Shadar-Kai do other races recognize what you are?
If not, are you simply seen as a drow or darker elf?
As a Shadar-Kai, are you open about your identity and the Raven Queen?
For most people, Shadar-Kai might just look like pale goth elves with a penchant for dreadlocks and chain-wear. There are likely some who might recognize the Shadowfell elves for what they are, and it's doubtful that if asked, the Shadar-Kai would hide their patron's identity.

From what I've read, I'd say other elves would know and possibly other races if they've looked into the Raven Queen and her followers. Shadar-Kai wouldn't shy away from who or what they are because it sounds like they a proud to serve the Raven Queen, even if she scares them. Ultimately, the DM would have final say in who recognizes the Shadar-Kai and who wouldn't, and what their overall personality is. As a player you would have the power to decide if you are open about yourself or closed off and shy away from the topic. That is the beautiful way that DnD is, in that in the end you make the world and it's creatures your own. Rules are only guidelines and lore can be changed to fit the story. Wizards of the Coast worded the books they way that they did for that very purpose, so we can make it our own. Best I can say is, talk to your DM and see what they have to say on the matter. If you are the DM then it's however you see it.

In a Nentir Vale game, Shadar-kai are legends come to life for most people, but everyone knows their patron, and know at least their name and basic affiliation.
In FR, less than a decade ago SK were primarily Netherese, with some pockets in Imaskar IIRC, and in the Shadowfell. Most ppl would assume you’re part of the Netherese diaspora, I’d think.
In Eberron, it would depend on the DM.
In my Eberron games, scholars know about the people of Vaes Shadar, and the Three Sisters they serve (The Raven Queen, The Red Witch, and the Silver Lady or Lady of Silver Mist). They’re related to elves, but so distantly that many scholars doubt it’s even true. They’re related to Vryloka, which have two subraces. Thematically, the three races are actually one race. SK live primarily in places within Thelanis that resemble both Ravenloft and the Shadowfell. And kinda The Dark Crystal.
In a Dark Sun game they might be nocturnal scavengers and hunters, silent and enigmatic, blessed with a reduced need for food and water by their now long dead Queen. Think nocturnal more violent Maji from The Mummy who are also carrion eaters.
