I want to make a character that will gross people out, not sure if this is the right place to put this though. I want a character that has a lot of body horror and will creep everyone out, I don't know how I should do this and if there is a better place to put this let me know.
This seems like a talk to your DM thing. There can be a lot of ways to do physically repulsive. Like, are you a member of a species which other creatures find gross, or were you disfigured somehow? And different races/cultures/individuals will find different things repulsive, so I don't know that there will be a universally applicable answer. Really, you will probably just end up picking one of the standard races, and say you look ugly, and just handle it through role play.
So, I'd suggest working just with your DM. If you want to go an extra step, maybe check out the Tasha's rules for a custom race to create what you're looking for.
I am looking for body horror type stuff, a character who does things in the most strange and gross ways, like moving in ways that should break bones constantly, not ugly, just off putting.
Mechanically, the easiest way to do this is with the Undead Warlock... you get the Form of Dread ability, which you can flavor with all kinds of fun body horror descriptions. The feature gives you temp HP and also lets you give your enemies the Frightened condition just by attacking them, which seems to pretty effectively reflect someone twisting their arm at an impossible angle to release an Eldritch Blast.
Okay, idea: you're a Changeling, but you have some kind of disorder that messes with your shapechanging, the process creating tumors and other unsightly and disturbing malformations regardless of the form you take.
Another idea could be a Simic Hybrid (or some equivalent chimeric experiment) whose "enhancements" came out... wrong. This has the added bonus of you becoming even more grotesque at level 5.

I agree with Transmorpher that Warlock is probably the way to go, though I wouldn't say that the Undead Warlock is necessarily better at being "ugly" (very subjective term) than the Fiend or most other Warlocks. I suggest taking Cloak of the Flies as an Invocation. CoF makes it so that being close to you literally hurts other creatures.
I mostly just suggested Undead because of the Form of Dread feature... it's something that involves a physical transformation for the character that is extremely open to whatever the player wants it to look like, and it outputs the Frightened condition. Cloak of Flies is another good feature to accomplish a similar effect, but it has the downside that you can't use it until you finish a short rest (which, to be fair, as a Warlock you're going to be trying to do regularly anyway) whereas Form of Dread gets uses equal to your Proficiency Bonus, which makes it easier to use repeatedly.
I suppose there's no reason you can't double down on the whole concept, like with playing as a Changeling in order to facilitate some pretty wild displays of grotesque imagery without requiring a resource of any kind. Form of Dread is really only useful in combat, while Cloak of Flies can help in social encounters or combat.
I don't know that imposing Frightened is necessarily the same as being ugly, but I see your point.
If the person who posted this question really wants to get into using Frighten as a mechanic, I would also suggest taking Fizban's Dragonborn as the race and taking the feat called Dragon Fear. This would replace the default breath weapon of the Dragonborn with ability to impose the Frightened condition on nearby creatures multiple times a day. However, whether this makes your character "ugly" or just awesome depends on your role-playing and the DM's willingness to go along with it.
I did change my mind to make them a fallen aasimar undead warlock also this is not to be ugly, rather to be creepy and terrifying and i finished the character and thanks to everyone who helped. Also I can't get cloak of flies until 5th level.
Someone has been watching Critical Role campaign 3. That said, this is something you should talk to your DM with. And possibly the players in your group to see what they are comfortable to be uncomfortable with.
There's a lot of stuff in Van Richten's Guide to Ravenloft that could fit the bill. Maybe a Reborn that died a horrible death but carried on, or a creature with the "Second Skin" or "Sybiotic Being" dark gift. Undead Warlock with Form of Dread is another good addition to this.
However, be wary of anything where "disfigured" means "ugly/horror". That can be quite insensitive to some, and is a trope that is best left in the waste bin.

My char did end up having a dark gift as an undead warlock but this was resolved, though i do not know how to mark it that way
Warlock gets an invocation called Cloak of Flies in Xanathar's Guide to Everything, which sounds great for this. You might just leave it active 24/7 lol.
You should check with your group to make sure you're not pushing any boundaries with them. I know a player who's really put off by bugs, even just descriptions of bugs in a role-playing game, so I wouldn't use this with them around, for example. Or I'd describe it as something else, like the smell of rotting flesh maybe.

I just now realized that Cloak of Flies can be left on indefinitely. Even though I've read the feature a few times even during this thread, in my head I just assumed it must have some kind of time limit. But it seems as long as you don't mind never being able to get within 5 feet of someone else without your bugs biting the crap out of them, there's no reason you can't wake up, summon the flies, then just keep the aura up all day long.
Yeah baby! It's one of only four stance swaps in the game! (The others are Wild Shape, Rage and Tenser's Transformation.) (Bladesong doesn't count because there's no drawback. Armor Model doesn't count because it's a rest feature.) And it's the only one that doesn't have a duration.
