I know there’s that one Warlock invocation for Pact of the Tome, and also Undead Warlock (damn Warlocks hate breathing huh) but surely that’s not all, right? Lookin’ preferably for early-level options.
If you’re looking for something more permanent, there’s some magic items (necklace of adaptation, breathing bubble, probably others) and spells — though the spells aren’t permanent.
If you’re the DM, you can always homebrew something to allow it.
You can be an Air Genasi. They can hold their breath indefinitely.
Is Air Genasi the only living, fully organic race that doesn't need to breathe if they don't want to?
I know that I played as a Tortle in one campaign, and they can hold their breath for a full hour. It didn't come up often, but any time it did the hour duration was more than long enough that it never became a challenge to hurry and get some air or something before it became a problem.
The autognome does not have to breathe, but they are similar to warforged. If you do not want to be warforged, you probably do not want to be an autognome.
The dhampir are technically still alive, but are kind of half-vampire and do not need to breathe.
Plasmoid canhold their breathe for one hour, like the tortle.
I assume you do not care about the many amphibious creatures.
While not explicitly stated as "does not need to breathe" the spell feign death lasts for 1 hour without concentration. The target:
"appears dead to all outward inspection and to spells used to determine the target's status. The target is blinded and incapacitated, and its speed drops to 0."
This makes me think that it can make a creature not breathe for an hour and still remain alive.
The sequester spell is an even more extreme version of this. The target:
"falls into a state of suspended animation. Time ceases to flow for it, and it doesn't grow old."
This also implies that the target does not need to breathe since time has stopped for it.
