So I know changelings can only shift between medium and small races.
My understanding is kind of limited beyond that. I'm currently playing a changling in my first campaign and they've been hiding out as a Tiefling in this party they've joined.
Basically my question is would my tail and horns be a physical part of my body that could be grabbed, used as a playful tool, ect? Or are they just minor illusions that might go though things if they happened to hit them.

The change is physical (e.g. a Medium Changeling who makes themself into a Halfling will be actually Small, allowing them to squeeze into a Tiny opening for example.) However, mechanical race features don't carry over. For example, a Changeling who becomes a Lizardfolk won't have thick skin or a bite attack, and one who becomes an Aarakocra won't be able to fly.
Per the Race's Description as follows, it states "you can make yourself appear", as in, you are changing your body structure, to match the chosen form, not casting an illusion, it is a physical change:

They're physical changes not illusions.
