Nurgul is giving a trial to some of his followers. As in a legal, guilty-or-innocent kinda trial.
The Fathomless is in the wasteland.
Kagorax is busy being a deity of fear, making nightmares, corrupting land, ect, ect.
Balatro teleports into Plague Keep, still disguised, thinking that his cult might take hold better in a place like this.
The first thing that happens is a large warrior holding a larger hammer halts him. "No further. You have no blessings of the Father of Pestilence."
“Au contraire, good sir. I am the victim of a horrific flesh-eating parasite. It has ruined my right leg, requiring the use of my cane.” He lifts his cloak slightly, revealing a bloody foot with almost no flesh left on it, with bone clearly visible.
"The warrior looks at the leg for a moment. "A great creation of His. Though unfortunate that it has hobbled you in this way. You may continue if you wish. Though keep your head down. You'll survive longer that way."
“Of course, brave sir.” He slowly makes his way down the road. “What an idiot…” he mutters, his face slowly turning skeletal.
Nobody seems to notice him. However, if he looks carefully, he can see certain people with an odd mark on their necks or wrists who stare at him unnervingly. The mark is in the spoiler.
“Wow, this could be a tough crowd to start a cult in. But there’s gotta be some folks here who are a little ‘sick’ of living like this...” He faintly chuckles at his own bad joke.
"There aren't." Comes a tell-tale voice from behind him.
“Ah. Can’t say that was entirely unexpected. Were you, by any chance, attracted by my genius comedy?” He slowly turns to face the voice’s owner.
Nurgul's hulking form looms over Balatro. "Do you think I cannot detect teleportation and the presence of other greater wills within the walls of my own city?"

Nurgul stares hard at balatro. "You try too hard. WAY too hard." He then takes something from behind his back. A pair of manacles apparently covered in flies. Before Balatro can react, he slams them on Balatro's wrists and closes them. Several large, very strong, very heavily armed warriors begin to march baltro in the direction of what must be a prison. "You await trial. Spend your days wisely."
Balatro has his ability to influence the environment and teleport restricted while he wears the manacles and they require a significant amount of power points to take off.
I gotta go, but Balatro's trial will be fun.

As she approaches, stepping past the threshold of the town line the sun seems to disappear, leaving a clear starry sky and the moon full and high. She sees several humans going about their daily business. Most seem to be ashen skinned or simply pale, with guards dress in black shadowy armor with a symbol of a black void with many violet dragon yes staring out of it. Mingled with the humans seems to be several shadows, undead specters harder in shrouds of pure blackness with back skeletal faces with ice blue energy peaking through the eye sockets and mouth.
One of the shadows would stop and slowly turn to Aria’s approaching form and stare intently….before raising its arm and waving at her in greeting.
