Zuli, you are pinned under the large stone block. It's painful, to say the least, but you're definitely not dead.
Arthilas and Darklis, with your combined efforts you are able to lift the stone block up to about shoulder height and hold it there comfortably. The only problem... In order for anyone to be able to go through, you have to stay there and hold it up. If one of you leaves the DC to keep it up will increase as well!
Agnomally, there is indeed a small gap on the floor. It doesn't seem like the stone was able to fall the whole way down, and the gap is only made wider because of Arthilas' and Darklis' efforts. It's to about their shoulders, so I doubt you'd need to polymorph into anything to walk/crawl through.

This post has potentially manipulated dice roll results.
"OOOOOWWWWWCHHHH!" Zuli yells. "Eh, just kidding. It's not that bad..." Pinned under the bolder, she attempts to roll over and push upward with her arms and legs to help lift the thing up and hopefully to get out herself.
Clementine grabs a bottle tied to her coat filled with what appears to be yellow-ish liquid and eyeballs, then teleports through to the other side of the stone.
(I swear this isn't dead!! Sorry for taking ages.)
Everyone that rolled strength, you all succeed and your combined efforts have made the stone significantly easier to carry. In case this is something you want to do, it's become light enough between the four of you that you can lift it up enough to shift positions and even walk under it---so long as you continue to hold it up. Zuli, you're no longer pinned and can move wherever you'd like.
Clementine, you teleport to the other side of the stone block, and from what you can see there is a small, 4' by 8' rectangle carved out on the floor. Inside the rectangle, there is a small chest. If you want to stay out from underneath the stone wall, you have to step into the rectangle, or risk getting squashed. But for now the rest of your cohort is holding it up pretty well.
Agnomally, where do you want to go? If you want to go with Clementine, you notice the same thing.

I think my fly is still active, since it lasts ten minutes. I’ll fly above the rectangle- getting close to the chest without touching down.
I agree about getting rid of the rock.
“I assume this chest is trapped or something… we should probably have one person in here and the rest hood up the rock, then have the inside person grab the chest and run. I think agnomally makes the most sense, I’m out of teleports”

"Oh yeah, let's do that!" Agnomally waits for the others to get into position and then takes the chest like Clementine suggested.
Darklis also continues holding the rock. She has a tired expression, though she shows no signs of giving under it's weight. She's just considering this boulder to be the physical embodiment of all her mental burdens... which doesn't make it feel any lighter.
Zuli rolls out from under the rock. "I can grab it if no one else wants to... or not. That's fine... Oh, you guys need help with that?" She crawls under the rock again and moves to help hold it up.
(Lol, how are 5 of us managing to fit in a doorway well enough to all help hold up a big rock? Do we somehow have the UA fairy’s Fey Passage or whatever it was called? The wonders of D&D….)
(Well, the rock is pretty big. If you're standing outside the doorway, and in the hallway, you're out from under the wall, which is more like a cube anyways. If you're inside the room, which is about 15'x15', then all but a small rectangle in the far wall of the room is covered by the stone. So maybe one or two of you would have to move under the rock, which I think that's where Zuli is, but the rest of you should be able to lift it from the doorway if you're close enough. Like shoulder-to-shoulder close enough.
Also, how are y'all gonna get rid of the rock?)
Agnomally, you're easily able to grab the chest and run! It seems to be filled with something, cause it's pretty heavy. If you'd like, you can make an Athletics check.
Either way, if you do or if you don't, make a Dex Saving Throw! And you too Clementine, Zuli, and anyone else ≈10 feet or less from the edge of the rectangle, assuming you haven't moved from where you're standing. If you're holding up the rock, roll with disadvantage if you try to move while still holding it up. You can drop it, but it'll make it harder for the others to hold it up.

I vote we try to move the rock out of the way entirely.

Arthilas continues holding the rock above his head. He just imagines it being a big, heavy barbell.
Darklis also continues holding the rock. She has a tired expression, though she shows no signs of giving under it's weight. She's just considering this boulder to be the physical embodiment of all her mental burdens... which doesn't make it feel any lighter.
Clementine, not holding the rock, considers it a large rock.
(Oh geez, I keep overlooking this thread.)
Zuli rolls out from under the rock. "I can grab it if no one else wants to... or not. That's fine... Oh, you guys need help with that?" She crawls under the rock again and moves to help hold it up.
