The Avatar of Plague has made its lair in a once beautiful but now withered garden in the center of the ruined City of Onyx.

Lair Actions

On initiative count 20 (losing initiative ties), the Avatar of Plague takes a lair action to cause one of the following effects; the Avatar of Plague can’t use the same effect two rounds in a row:

  • A 10 foot radius circle of foul smelling, slimy tentacles erupt out of the ground in an unnocupied space. The area is difficult terrain, and creatures that start their turn in the area take 2d8 necrotic damage, and creatures that end their turn in the area must make a DC 15 Dexterity saving throw or take 2d6 bludgeoning damage and become grappled as the tentacles try to crush them. The circle disappears after 2 rounds.
  • .A 10 foot radius sphere of paralyzing spores appear. A creature which starts its turn in the area must make a DC 13 saving throw or be placed in a trance. While in the trance, the creature is paralyzed for one round or until another creature breaks them out of the trance.

Regional Effects

The region containing the Avatar of Plague’s lair is blighted and sickly, which creates one or more of the following effects:

  • Food and drink that can be found within 6 miles of the lair looks fine, but is blighted and evil. If a creature eats the food, it takes 1d4 poison damage and must a DC 12 Constitution saving throw or become poisoned for 1 hour.

If the Avatar of Plague dies, these effects fade over the course of 2d4 days.
